This PR cleans up the source code style and documentation:

• Function parameter wrapping changed to always put params on a new line, reducing longest line length dramatically and improving readability as they now use consistent columns for all functions. This change also removes vestigial clang-format leftovers now we are no longer using it.
• Cleaned up Doxygen formating, making consistent use of whitespace and refactoring any documentation line over 100 characters.

Change set is large, but majority of the changes are simply mechanical transforms or autogenerated by the layer code generation tooling.
