DISCLOSURE: LLM-GENERATED TEXT

Why: A shared OpenCode service started with empty credential exports 401s the remote github and zai-web-search MCP servers for every session while .env.local, shells, and endpoints stay healthy.

What:

  • Recycle a service holding present-but-empty credentials
  • Absent keys stay unjudged, so fail-safe is unchanged
  • Warm the service with .env.local-loaded environment
  • mcp:doctor reports the poisoned service with remedy
  • Document the remoteEnv empty-string trap at its source

Fixes the 2026-10-31 incident: service pid held GITHUB_PERSONAL_ACCESS_TOKEN= and ZAI_API_KEY= (VS Code remoteEnv resolves an unset host variable to an empty string); both servers were absent from sessions although probes returned HTTP 200.

Walkthrough

Startup now warms OpenCode with credentials loaded from .env.local. The service recycler detects present-but-empty credentials as mismatches. mcp:doctor checks shared-service state and reports credential status.

Changes: OpenCode credential handling

Layer / File(s) Summary
Load credentials for service startup
.devcontainer/devcontainer.json, .devcontainer/post_start.py, scripts/tests/test_devcontainer_lifecycle.py
post_start.py passes the .env.local environment to both OpenCode warm-up commands. The lifecycle test checks that the loaded token is used when the inherited token is empty.
Detect and recycle stale credentials
.devcontainer/recycle_stale_opencode_service.py, scripts/tests/test_opencode_service_recycle.py
The recycler retains empty service values and treats them as stale when the corresponding file value is non-empty. Tests cover divergent and empty credential combinations.
Report shared-service credential status
scripts/mcp_doctor.py, .llm/mcp/README.md, scripts/tests/test_mcp_credentials.py
mcp:doctor runs a dry-run service check and reports stale credentials, dead processes, errors, or matching credentials. Documentation and tests cover poisoned and healthy service states.

Estimated code review effort: 3 (Moderate) | ~20 minutes

Merge Risk: Low · previous review 4f043

The credential-recycling and startup changes look sound. The doctor's remedy text can point operators at the wrong service record when XDG_STATE_HOME is set, and could advise stopping a process that reused a stale PID. Both are narrow diagnostic-guidance issues and are worth fixing before or soon after merge.

Security architecture risk: Moderate · up to 4f043

Reloading repaired credentials improves shared-service startup. However, the expanded restart rule trusts a recorded process ID without confirming that it still belongs to OpenCode, so a stale record could cause an unrelated process to be stopped. Recovery instructions can also point to a different state directory from the one inspected.

Retained concerns

  • Medium · security · inferred: Empty credentials now authorize termination without establishing service identity. The recycler accepts a live recorded PID and differing tracked environment values, then signals that PID without checking executable identity or process start time. If a stale record points to a reused PID carrying empty forwarded credentials, an unrelated process can now satisfy the termination predicate. PID-only targeting existed at the base, but empty values previously could not trigger it.
  • Low · reliability · inferred: The new unjudged-service recovery instruction is not bound to the resource inspected. Inspection honors XDG_STATE_HOME, but the instruction directs the operator to stop the PID in the hardcoded ~/.local/state record. With a nondefault state directory, this can target a different process while leaving the inspected service unchanged. Even at the default path, the instruction does not establish that the recorded PID still belongs to OpenCode. This is operator-mediated recovery risk, not automated termination by the doctor.
Security review details

Security Blast Radius

  • inferred — A recycling invocation targets one recorded PID. For the genuine shared service, termination affects its attached sessions; for a stale or reused PID, another process can be affected. Effective exposure is limited by the caller’s /proc visibility and signaling permissions, normally the same user in this devcontainer integration. Broader tenant or host authority was not established.

Security Findings and Attack Paths

  • inferred — The supported failure path is a stale discovery record naming a live unrelated process, that process carrying an empty tracked credential, and .env.local containing a nonempty counterpart. The head then permits signaling where the base did not. Deliberate targeting would additionally require control over the selected record or its lifecycle; lower-trust write access to that record was not established.

Trust Boundaries and Controls

  • observed — The doctor crosses from diagnostics into local process inspection through fixed subprocess arguments and --dry-run. That mode returns before signaling or record deletion. Missing file evidence, unreadable process environments, absent tracked keys, and empty file values do not authorize automated termination, but these controls do not authenticate the live process selected by a PID.

Resilience and Maintainability Implications

  • observed — Reloading the credential file after repair prevents both warm-up commands from retaining the rejected pre-repair value. Matching services remain untouched, and dry-run preserves the process and discovery record. These are meaningful countercontrols, although they do not resolve process-identity or concurrent record-replacement risks.

Hardening Proposals

  • proposed — Bind termination to a verified service identity or service-owned stop operation, serialize competing lifecycle transitions, and verify that the discovery record still represents the same service before deleting it. Recovery instructions should identify the resolved state record and require identity verification before an operator stops its PID.

Three review findings on the credential recycle path: the warm attach could
use the pre-repair environment, mcp:doctor claimed a match it never
compared, and the service fixtures ran on hosts without /proc. The recycler
now names the keys it compared, the doctor reports an unjudged service, and
post_start reloads .env.local before warming the singleton.

Comment thread scripts/mcp_doctor.py Resolved
VS Code devcontainers resolve an unset ${localEnv:VAR} in remoteEnv to
an empty string, so a shared opencode service started outside a loaded
shell exports GITHUB_PERSONAL_ACCESS_TOKEN= / ZAI_API_KEY= and expands
every {env:VAR} reference in the generated configs to no credential:
the remote github and zai-web-search MCP servers 401 for every session
while .env.local, fresh shells, and the endpoints all stay healthy.

The recycler ignored present-but-empty service credentials because it
could not distinguish them from an unreadable /proc environ. Parse them
now and judge a present-but-empty value against the file; an absent key
stays unjudged so the fail-safe property is unchanged. Document the
remoteEnv trap where the exports originate.

Evidence on 2026-10-31 (service pid 2241): environ held both canonical
names empty with alternates and UNITY_MCP_TOKEN set; the old recycler
reported nothing for the same live state.
post_start warms the shared opencode service through 'opencode mcp
list', and that attach starts the service when none is running. It ran
on the raw lifecycle environment, which carries the remoteEnv-forwarded
empty credential strings, so the service it spawned 401'd the remote
MCP servers for its whole life. Pass the .env.local-loaded environment
that main() already builds for its other children.

Contract test: an exported-empty canonical credential in the lifecycle
environment plus a file value must reach the warm attach as the file
value.
The 2026-10-31 incident went undetected because npm run mcp:doctor
probed credentials from a fresh shell: endpoints answered HTTP 200,
stdio servers spawned, and every check passed while the shared service
behind every session held empty canonical credentials. The doctor now
runs the recycler in --dry-run and turns its finding into a failure
that names the keys (never values) and the remedy. Skips gracefully
off Linux and when no service is running.

Document the empty-credential poison shape and the new detection in
the MCP README alongside the existing rotation-caching guidance.

use the pre-repair environment, mcp:doctor claimed a match it never

The doctor told operators to rerun the recycler when the service named no
tracked credential, but that path changes nothing: the service is unjudged,
not stale. The recycler now separates a missing file key from a service that
names no credential, and the doctor asks for a restart from a loaded shell.

A discovery record can outlive its service and the operating system reuses
pids, so a live unrelated process could be judged and then named to the

the opencode service and treats a mismatch as a stale record; the doctor
reports the resolved record path and never issues the stop remedy for it.
