docs: remove error-red terminal screenshots - #23

Replace the dry-run screenshot with a pure create plan (exit 0), drop the init-conflict and invalid-config shots from all pages, and rewrite captions to keep every visual error-free. Factual description of exit codes 2/3 remains as text; the exit-codes page now features the green validate capture.
