Attribute whole-snapshot skill errors to the skill directory - #171

discoverSkills blamed every per-skill failure on <dir>/SKILL.md, so a
whole-directory failure ('skill snapshot exceeds 512 KiB') pointed at the
manifest. Snapshot-phase failures now report the skill directory, except
SKILL.md's own size/binary failures which stay on the manifest; manifest
content errors still report SKILL.md and directory-name mismatches report
the directory. No security behavior changed: refusal causes, messages, and
all size/symlink caps are untouched.

Part of #170 (path attribution; silencing mechanism still to decide, see proposal comment). Snapshot-phase failures now blame the skill dir; file-specific errors keep file paths. Behavior otherwise unchanged.
