feat(audio): say why exclusive output was refused - #836

Each exclusive backend now marks why it fell back to shared mode: another application holds the device (WASAPI device in use, ALSA busy after the reservation, CoreAudio hog mode taken), the system forbids exclusive use (WASAPI exclusive mode not allowed), or the device offers no usable format. The output layer stores the reason and the notice names it.
The notice has one sentence per reason, and the Settings card says what to do: close the other app, turn on the Windows exclusive-control option, or nothing when the device has no usable format. 17 locales.
A successful DoP open returned before the reset and kept an earlier PCM refusal.

The negotiation kept only the last attempt's error, so a busy device refused on format at the next layout read as a format problem. Every attempt is classified and the most useful cause wins; a device with no layout to open counts as format.
render_mode::apply ends on a needless return on macOS, where the software switches are compiled out. The sidecar test used exists() on a case-insensitive filesystem, where Song.lrc resolves to the user's song.LRC; it now lists the folder and checks the user's file is untouched.
