Adds a short Migrations section to AGENTS.md. Every API container runs migrate (pre_startup.sh) before it serves. An index build on concepts or mappings can outlast the health check, and an interrupted CREATE INDEX CONCURRENTLY leaves an INVALID index.

The note covers:

• the migration pattern OpenConceptLab/ocl_online#476 | Covering list indexes so deep pages walk their OFFSET in the index #938 uses;
• building by hand on staging and production before deploying;
• dropping a replaced index only after EXPLAIN shows the new one in use.
