* cni: amd-host-device: inject policy routes into IPAM for SBR chaining

Adds two optional NAD fields — nexthopNetAddrOffset and routeDstPrefixLen —
that allow users to inject an explicit route into the static IPAM result
passed to the chained SBR plugin. For /31 point-to-point links the nexthop
is derived via XOR of the peer bit; for all other prefix lengths it is
computed as the subnet network address plus the configured offset.
Removes the previous implicit /31 gateway injection from the addresses array.
