Generated CI workflow still runs bare npx charter (breaks under Yarn PnP / without a local install) #380

Found in the #379 review (#366).

The GitHub workflow that charter setup --ci github / charter bootstrap --ci github generates (getGithubWorkflow in packages/cli/src/commands/setup.ts) still runs Charter as a bare npx charter …. Under Yarn Plug'n'Play npx can't resolve the PnP-installed binary. And when the CLI isn't installed locally, the name charter resolves to an unrelated registry package (0.0.2, which has no bin).

#379 fixed this for the pre-commit hook by resolving the CLI through the repo's package manager. The generated workflow should do the same: emit yarn charter for Yarn, pnpm exec charter for pnpm, and an npx/npm exec form that names @stackbilt/cli for npm.

Changing the template changes its hash. Update the known-template table from #374 (#377) to match, and mark the new template as a safe refresh only if its permissions and triggers are unchanged.
