feat: re-anchor skill location on project switch - #1447

Appends "Skills are in your skills/ directory." to the /projects select and clear silent messages so the agent doesn't lose track of where skills live after switching projects. Adds a Tool Call Discipline rule against parallel edits to the same file.

Appends "Skills are in your skills/ directory." to the /projects select and clear silent re-anchor messages so the agent doesn't lose track of where skills live after switching projects. Adds a Tool Call Discipline rule against parallel edits to the same file, and renumbers the downstream rules to keep the sequence coherent.
