Summary

• Reports one OSC 7501 root record for the session: blocked while a gate is outstanding, error when the turn is stalled, working while a task or agent is live, otherwise idle.
• Pings the operator once when a permission or operator gate reaches the screen.
• Reads clipboard images through the OpenTUI clipboard service instead of a macOS-only osascript paste. Image rendering and audio stay out of this change.
