Visible changes for consumers

• The orange badge tones (accent, warning, access) use an opaque pale fill and two new theme tokens, --primary-tint and --primary-tint-foreground. A theme that overrides the full token set needs both.
• The closed model and mode pickers are named "Model, Deep" (label plus choice), so a lookup by the name "Model" alone no longer matches.
• labels.running and labels.held receive a done count that excludes failed calls.
• The package ships a skills/ directory, built by a new build:references step.
