Three README sentences narrowed to what the starters show on the published 0.1.x packages (each starter's own npm test runs them on Node 22 and 24 in CI).

File Before After
hl7-to-fhir/README.md a Patient from PID and an Encounter from PV1, following the maps of the HL7 Version 2 to FHIR implementation guide ..., grounded on the maps of the HL7 Version 2 to FHIR implementation guide
cli-tour/README.md The drop is reported as a finding, not hidden. The drop is reported as a finding.
x12-eligibility/README.md serializeX12 turns it into the wire text a payer sends. serializeX12 turns it into wire text.

"Grounded on" is the wording the @cosyte/transform README uses for its relation to the guide. No code, expected output or test changes.

- hl7-to-fhir: the transform builds the Bundle "grounded on" the maps of
the HL7 Version 2 to FHIR implementation guide (was "following").
- cli-tour: "The drop is reported as a finding." (drops "not hidden").
- x12-eligibility: serializeX12 "turns it into wire text" (was "the wire
  text a payer sends").
