Closed as not planned

Description

Unhardened Defect PR Audit Finding

Defect Pull Request #3467 was merged without adding guardrails or lessons to any declared hardening surface.

Declared Hardening Surfaces

  • docs/TESTING.md
  • ARCHITECTURE.md
  • SECURITY.md
  • CONTRIBUTING.md
  • src/devops_cli/ai/tasks/*.md
  • src/devops_cli/ai/knowledge_base/
  • .devops/review.toml
  • src/devops_cli/review/rules/

Per project invariants (AGENTS.md), defect fixes must codify root-cause guardrails into at least one declared surface to prevent recurrent regressions.

Intake placed this item to the backlog: intake puts every new item in the backlog. A person plans the milestones.

    • Type: type/bug. The fix prefix of the title gives the type.
    • Scope: none. The title has the scope hardening, and .github/labels.yml declares no scope/hardening label. Add the label to the file to give the scope to this item.
    • Priority: P2-Medium. A process compliance gap that risks recurrent regressions but does not block users or expose a live vulnerability.
    • Value: Medium. Closes a gap in the project's regression-prevention invariant for one specific PR.
    • Effort: Low. Requires adding a guardrail entry to one or more existing declared surface files.
    • Related: fix(hardening): defect PR #3452 missing declared surface hardening #3459, which asks for a different change. Both are audit findings for unhardened defect PRs, but each names a different PR and requires its own root-cause guardrail entry.

PR #3467 already holds a guard for its defect: tests/ai/mcp/test_path_confinement.py, and a delta under openspec/specs. This item holds only the audit template, with no root cause and no work that a person or a job can plan. docs/SELF_IMPROVEMENT.md:165-168 says that a check that fails when the problem returns guards the problem, and that a rule goes into a document only when no check is possible. So this item closes as not planned. It is not a duplicate: it names its own pull request.

#3479 owns the cause: the audit counts only the declared documents, and it files one issue for each pull request. #3479 lists this item and this pull request as evidence.
