Closes #1491 — AI-3792

What & why

The server holds only the snapshot declare_plan_document sent, so a document the agent edits after declaring it stays stale in the desktop app. The plans MCP server now records each accepted declaration locally with all its arguments. A new synchronous Claude PostToolUse hook on Write|Edit|MultiEdit (kcap hook --claude --plan-edit) declares a recorded document again with its current text, to the server that accepted it. The tool description and the SessionStart plans nudge now ask the agent to declare again after a change, which covers other harnesses and shell or patch edits.

Where to look

  • The replay carries every original argument because the server overwrites what a later declaration omits.
  • The hook is synchronous, and holds a per-document file lock while it reads and posts; declare_plan_document takes the same lock (or returns a retryable error if it cannot), so parallel edits and re-declarations post in turn with the newest text and arguments last. SessionEnd keeps the ledger because a resumed session keeps its id.
  • The app shows the new text only once the matching kcap-server change lands: the server ignores a declared snapshot while its own transcript reconstruction has a complete revision.

Verification

  • DeclaredDocumentLedgerTests, McpPlansServerTests, ClaudeHookCommandTests: 7 + 41 + 115 passed. The two lock tests fail with the lock, or the re-read under it, removed.
  • Full CLI unit suite on the first commit: 6672 run, 1 failure, session_start_posts_to_session_start_route, which passes 3/3 alone.
  • dotnet publish -c Release: no IL2xxx/IL3xxx warnings.

🤖 Generated with Claude Code

Summary by CodeRabbit

  • New Features
    • In Claude Code, declared plan, spec, and design documents refresh automatically after edits made with Write, Edit, or MultiEdit. The refreshed declaration uses the document’s current content and retains its declaration metadata.
    • Redeclaring a document replaces its stored copy with the current text. Declarations are stored locally and associated with the session and server that accepted them.
    • Plan guidance asks agents to redeclare documents after changes, and task creation no longer requires plans to have discrete steps.
  • Documentation
    • Clarified automatic refresh behavior and noted that other harnesses and edit methods require manual redeclaration.

The server keeps only the snapshot sent at declaration, and overwrites any
field a later declaration omits, so the replay carries every original argument.

📝 Walkthrough

The CLI records accepted plan document declarations per session. A Claude PostToolUse hook redeclares a matching document after Write, Edit, or MultiEdit, using its current text and stored declaration arguments.

The hook handles Write, Edit, and MultiEdit events. For a matching declaration, it posts the document’s current text and stored metadata. Tests cover declaration matching, locking, server matching, and resumed sessions. The README and plans nudge describe redeclaration and the hook’s supported edits and limits.

Merge Risk: 🔵 Low · up to 2f3fa

Plan text can remain stale after a failed refresh, and a declaration can fail during a concurrent edit. The refresh has a manual recovery path; making document reads resilient would reduce the remaining merge risk.

Security architecture risk: 🟡 Moderate · up to 2f3fa

Automatic refresh retains important path, session, and server checks. However, refreshes are tied to the server address rather than the account that originally declared the document. Changing accounts on the same server can therefore change the credentials used for an automatic upload; whether the server prevents cross-account association remains unverified.

Retained concerns

  • Medium · security · inferred: Automatic replay preserves the accepting server address but not authenticated ownership. A document declared under account A can remain eligible after a later hook resolves account B on the same URL. The hook can then submit the document with B's credentials without another explicit declaration. Server-side rejection or ownership enforcement was not established, so cross-account association remains an unresolved security boundary rather than a verified disclosure.
Security review details

Security Blast Radius

  • inferred — The demonstrated local exposure is automatic submission of eligible, previously declared documents under the same server URL. The account-change scenario requires a changed authenticated context and a matching edit/session, not merely an unauthenticated remote request. No arbitrary destination selection or verified cross-tenant acceptance was established.

Security Findings and Attack Paths

  • inferred — The unresolved path is: declare under account A, resolve a later hook under account B at the same URL, then edit the recorded document. URL-only lookup permits replay using the later credential context. Unlike the base's explicit declaration path, this needs no new declaration decision. Whether the endpoint rejects the session/account mismatch remains a missing ownership proof.

Trust Boundaries and Controls

  • observed — The hook checks disabled-session and repository/path capture exclusions before posting. Ledger lookup rejects a different server URL, and token resolution separately withholds credentials bound to another server. These are meaningful controls, but neither establishes that a same-URL refresh uses the original authenticated account.

Hardening Proposals

  • proposed — Bind replay eligibility to stable authenticated account or tenant identity as well as server origin, and require explicit redeclaration when ownership changes. Verify the endpoint's session-ownership enforcement for same-server account changes rather than relying on URL equality.

High-Level Assessment

The following are alternative approaches to this PR:

1. Rely on agents to re-declare manually
  • ➕ Requires no local ledger or edit hook.
  • ➕ Works for edits made through any tool or harness.
  • ➖ Leaves snapshots stale when agents forget to call the tool again.
2. Watch declared files for changes
  • ➕ Could detect edits made by shell commands and other harnesses.
  • ➖ Requires persistent watcher lifecycle management and may produce declarations unrelated to agent edits.

Recommendation: Keep the targeted hook and ledger: they refresh observable Claude edits without a persistent watcher, while the agent prompts cover edits the hook cannot see. Manual-only refresh would not reliably address the stale-snapshot problem.

P2 Badge Preserve declarations when Claude sessions resume

A Claude SessionEnd is not necessarily the final use of a session ID: claude --continue/--resume reopens the same transcript and session. Evicting the declaration ledger here means that, after such a resume, edits to previously declared documents are no longer refreshed because HandlePlanEditCore cannot find them. Retain these entries for the existing age-based retention period, or restore them when the same session starts again.

Fixed in e97cadf: SessionEnd no longer evicts the ledger, so a resumed session (same id) keeps refreshing. Entries are reaped 30 days after the session last declared or edited one.

P2 Badge Serialize refreshes for repeated edits

Because this PostToolUse hook is asynchronous, two quick edits to the same declared document launch independent processes that can complete out of order. Each process snapshots the file before sending an unconditional replacement, so a slower request containing the first edit can arrive after the request containing the second edit and leave the server with stale text. Serialize refreshes per session/document, or otherwise ensure older snapshots cannot overwrite newer ones.

Fixed in e97cadf: the plan-edit hook is now synchronous (3 s, like the existing --task-list hook on Write|Edit). Claude waits for each refresh before its next step, so refreshes reach the server in edit order and complete before a headless teardown.

P2 Badge Normalize paths before hashing ledger keys

On case-insensitive Windows and default macOS filesystems, two differently cased absolute paths can name the same file, but this hashes the path using case-sensitive UTF-8 bytes. If a document is declared as docs/Plan.md and Claude later edits docs/plan.md, Find computes a different entry key and silently skips the refresh even though both paths resolve to the same file. Normalize according to the platform's path-comparison semantics before deriving the key.

Fixed in e97cadf: the ledger key is case-folded on macOS and Windows. On a case-sensitive volume a collision costs one extra re-declaration of the declared file.

Action required

1. Plan text can reach the wrong account ✓ Resolved
Description
DeclaredDocumentLedger records a declaration by session and path without the profile, account, or
server that accepted it, while HandlePlanEdit posts through the currently selected profile. If a
user switches profiles or servers during an active session, the next edit can upload that document's
current contents to the newly selected destination.

Inspect the response and retain retryable failures for a later bounded attempt; ensure retries cannot overwrite a newer snapshot.

6. Long sessions lose other declared files ✓ Resolved
Description
DeclaredDocumentLedger.Reap expires whole session directories by directory modification time,
while Record overwrites files without updating that timestamp. After a directory has aged past 30
days, recording one document can delete the other declarations in the still-active session before
writing the new entry.
Code

src/Capacitor.Cli/Plans/DeclaredDocumentLedger.cs[R43-46]

+        foreach (var dir in Directory.EnumerateDirectories(root)) {
+            try {
+                if (Directory.GetLastWriteTimeUtc(dir) < cutoff) Directory.Delete(dir, recursive: true);
+            } catch { }
Relevance

●●● Strong

This is a concrete local filesystem-retention bug: file writes do not refresh the directory age used
for reaping.

ⓘ Recommendations generated based on similar findings in past PRs

Evidence
Reaping deletes a directory based solely on its modification time; recording writes an entry file
but does not refresh the directory's timestamp.

7. Merge subject lacks issue reference ✗ Dismissed
Description
The PR title Declare a plan document again when the agent edits it does not end with a GitHub
issue reference in the required (#digits) form. When this title is used as the merge subject, it
cannot satisfy the repository's single-clause subject format with explicit issue linkage.
Code

README.md[136]

+- **Declared-document refresh (Claude Code)** — the server holds the copy of a document `declare_plan_document` sent, not the file. When the agent then changes a document the session declared with `Write`, `Edit` or `MultiEdit`, an async `PostToolUse` hook (`kcap hook --claude --plan-edit`) declares it again with the same arguments, so the server and the desktop app show the current text. The declarations it can replay are kept locally under `~/.config/kcap/declared-documents/` and dropped at session end. Other harnesses, and edits made any other way (a shell command, a patch tool), rely on the agent declaring the document again, which the tool description and the plans nudge ask for. It has no opt-out of its own.
Relevance

●●● Strong

Title-format compliance is a deterministic repository rule, and the PR title lacks the required
issue suffix.

ⓘ Recommendations generated based on similar findings in past PRs

Evidence
The checklist requires the commit or merge subject to be a single imperative clause ending in
(#<digits>), while the supplied PR title is `Declare a plan document again when the agent edits
it` and has no parenthesized GitHub issue reference.

Rule 2897961: Enforce single-clause imperative commit subject with GitHub issue reference and 80-char limit
README.md[136-136]

Agent prompt
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution

## Issue description
The merge subject is missing the required trailing GitHub issue reference in the `(#digits)` format.

## Fix Focus Areas
- README.md[136-136]

## Recommended Fix
Update the PR title or merge subject to retain the imperative wording and append the explicit GitHub issue reference, for example `Declare a plan document again when the agent edits it (#1491)`, if that is the correct GitHub issue number.

An async hook let two quick edits land out of order and was killed by a headless
teardown; session-end eviction broke a resumed session, which keeps its id.

🗄️ Data Integrity & Integration | 🟠 Major | 🏗️ Heavy lift

Keep a refresh pending when its POST fails.

If the documents endpoint returns 503, PostOnceAsync returns a response, but this handler discards its status and returns zero. No later hook retries this revision, so a single transient failure can leave the declared snapshot stale indefinitely without another edit or manual declaration. Check the response and retain a bounded pending refresh for a later attempt; keep the hook fail-open.

@coderabbitai On the outside-diff items from the last review:

  • Failed POST retry: not changing. A failed refresh is repaired by the next edit or by the agent re-declaring, which the tool description and plans nudge ask for; spooling a whole document for retry is more than this warrants.
  • Qualifying the claims: README now says the hook leaves the server holding the current text, which is true whatever the desktop app renders. The tool description is already accurate: re-declaring does replace the stored copy on the server.

@alexeyzimarev Thanks for clarifying.

  • I accept the decision not to add retries. A later edit or agent re-declaration can repair a failed refresh. No retry spool is needed for this PR.
  • I checked README.md and the tool description. Both describe the stored server copy, not what the desktop app renders. I withdraw the request for further qualification.

Learning: In kurrent-io/kcap-cli, distinguish a declared document's stored server copy from the desktop Artefacts rendering. Re-declaration replaces the stored server copy. Claims scoped to that copy in README.md and the declare_plan_document tool description do not need qualification about desktop rendering or the matching kcap-server reconstruction change.

An edit refresh that read the ledger before a concurrent re-declaration could
post the old arguments last and overwrite the newer server copy.
