docs: say where the theme skill reaches every provider - #97

Conversation
lich now copies skills/theme as lich-theme into opencode, oh-my-pi, Crush and Kiro CLI. The README names where each harness gets the skill, and CLAUDE.md records that lich fetches the skill's files by name, so a change to them starts in lich.

Summary
lichdotdev/lich#1005 makes lich copy
skills/themeaslich-themeinto opencode (where it becomes/lich-theme), oh-my-pi, Crush and Kiro CLI, the harnesses that read no skills out of a plugin.SKILL.md,template.json,validate.mjs) and rewritesname: theme, so adding, renaming or moving a file the skill needs, or renaming the skill, changes lich's list first. No script or contract change: nothing to release on its own.
