feat(skills): plan reviews before ready; merge and review reuse a counted record - #288
Closes two gaps in how the quest skills use the ## Review record:
quest-plan: it now opens its PR as a draft and runs /quest-review, like quest-start already does, instead of opening ready and going straight to /quest-merge with only "the quest should be reviewed before it is merged". Quests committed to an existing open PR are reviewed on that PR's next push, as the guide describes.
/quest-review: it skips the round when a ## Review record the guide counts names the committed head, or the guide's review exemptions cover the changes since the head it names, and nothing is left unrefuted. quest-iterate ends by calling /quest-review, so iterate no longer re-reviews a head that's already reviewed. Its existing rule of reviewing before every push still covers new heads.
The maintainer asked for this on 2026-10-30: plan and start run the reviews, while merge and iterate review only when the record isn't present.
Changes while merging (2026-11-01)
Merged origin/main (a27cacd). This PR's assets/skills/merge.md edit conflicted with moq-dev/quest#292 and moq-dev/quest#300. Those PRs rewrote /quest-merge to queue a head only when "a ## Review record the guide counts names it, or the guide's review exemptions cover the changes since the one it names", and otherwise to send it to /quest-iterate. That already covers this PR's merge.md change, so the conflict was resolved to main's text and this PR no longer touches merge.md.
CodeRabbit's thread on merge.md:50 asked for the review exemptions to be honored. main's merge.md already does that. The same gap existed in /quest-review's new shortcut, so that shortcut now honors the exemptions too.

Review
Reviewer: Codex gpt-6.1-sol at medium effort, via quest review --base origin/main, round 1 of a new session (01a198ee-7993-77f2-8d66-96d3f4f05036), started because AGENTS.md changed on the base.

Findings: none.
Earlier session (01a18d39), round 1 on 025224da's parent: the shortcut trusted any ## Review section, including one an outside contributor wrote. Fixed in 025224da: shortcuts require a record the guide counts.
…ord naming the head
quest-plan now opens its PR as a draft and runs /quest-review, like quest-start. quest-merge reviews the head locally when no ## Review record names it, and /quest-review (which quest-iterate ends with) runs no new round when the record already names the committed head.

Walkthrough
The skill instructions now define a workflow that opens a draft PR, reviews the branch, records ## Review, and marks the PR ready. They also specify when later pushes or the merge step require a new review, based on whether a qualifying review record names the checked-out head.
Merge Risk: Low · previous review 02522
Exempt changes may receive an unnecessary review round before landing; align the merge instructions with the guide.

I merged origin/main and resolved the merge.md conflict to main's text. moq-dev/quest#292 and moq-dev/quest#300 already make /quest-merge land only a head that a counted record or the review exemptions cover. That also answers CodeRabbit's thread.
/quest-review's new shortcut honors the review exemptions too.
