fix: rename leftover .json references to codev - #74

Config-rename stragglers: credential-dialog strings and toast in dialog-provider, model-not-found hint in tui error util, comments in dialog-configure and run-workspace-server, v2 config spec discovery prose, and the root .gitignore entry. DIVERGENCES.md config-rename row extended with the upstream files touched. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>
