Hand each quest's result to the user instead of letting agents decide alone.
spawn-quests: agents open draft PRs and list open decisions with recommendations; as each reports, the coordinator explains the result, prompts inline (a few decisions per prompt, recommendation first), offers follow-ups as a multi-select to /plan-quests, resumes the agent with each answer, and marks the PR ready once nothing is open. Agents switch into their own worktree before editing.start-quest: explains the result, prompts for every open decision, offers follow-ups, and keeps the PR a draft until the user confirms; a background agent lists decisions instead.Moved here from moq-dev/moq#4272 so the vendored skills carry it.
