Fix Codex prompt delivery and account-scoped quota status - #158

Automated prompts could remain unsubmitted or reach an occupied/startup composer. Bracket task pastes and wait for an empty composer belonging to the current launch. API-backed profiles appeared as protocol failures, while account/configuration changes could retain previous subscription observations. Detect API mode and invalidate cached limits when their source changes, preserving real protocol errors. Keep existing permissions, MCP authentication/discovery, result caps, wait/get workflow and cancellation behavior. Add generic delivery, account and UI regressions. Validation: full suite 2,241 passed, 6 skipped; final quota suite 14 passed. Typecheck, build and source/built-bundle secret audit passed.

Codex prompts could remain in the composer, mix with a person’s draft, or reach startup UI before the current launch was ready. Send automated tasks with explicit paste boundaries and wait for an empty composer in the same launch. Other providers retain their existing delivery bytes.
API-backed Codex profiles were reported as protocol errors, and cached subscription quotas could survive an account or provider-configuration change. Detect the account mode before reading subscription limits, show an API-profile explanation, and invalidate observations when their account/configuration changes.
Validation: full local suite — 2,241 passed, 6 skipped, no failures. The final focused quota suite also passed all 14 tests, including the added account/configuration failure regression. Local build, typecheck, UI rendering tests and source/built-bundle secret audit passed. The new delivery and API-profile tests reproduce the failures on unchanged main.
Supersedes #155, #156 and #157 with a smaller bugfix-only scope. Existing result limits, wait/get workflow, cancellation rules, MCP discovery/authentication, folder trust and permission policies are preserved. No OmniRoute quota integration is included. No installed application, profile, account or service was changed.
