Summary

Until now, the only thing between a runner-published head and main was the maintainer's earlier pick of merge. The inbox's direct merge and a merge job's land publication both went ahead on that pick, limited only by branch rules, and moq-dev/quest's main requires no reviews.

Both paths now call one gate, cloudflare/src/github/review.ts, before they land. It reads GitHub's own records, plus the author's own ## Review record as the guide's default policy counts it. A head lands when one of these holds:

  • GitHub enforces the rule itself: a ruleset on the base requires an approval, dismisses stale approvals or requires approval of the last push, and GitHub reports current_user_can_bypass: never for the App. In that case kix defers to GitHub.
  • A person approved that exact commit: a writer (latestOpinionatedReviews(writersOnly: true)) other than the PR's author approved it. Reviews by Apps or bots never count.
  • The App's check passed: a completed, successful quest review check run written by this App sits on that exact commit.
  • The author recorded their own review: the ## Review section of the PR body names that commit's full SHA, the author has write access (GET /collaborators/<login>/permission reports admin or write), and the body's latest edit is the author's (userContentEdits(first:1)). It never counts on a commit GitHub attributes to an App's bot (GET /commits/<sha> committer type Bot), as it does every commit a runner publishes, nor across a merge an App committed.

A review carries across merges of the default branch that resolved nothing, up to five merges back. kix checks this from the trees of the merge, both parents, and the merge base. Every path must hold the reviewed side's version or the default branch's, whichever side changed it. The merged-in parent must already be on the default branch.

Anything else blocks the landing. That includes a file both sides changed, any other commit, a truncated tree, or a failed read. When the head is blocked:

  • the inbox's Merge this PR is refused with the reason;
  • a merge job pauses on its existing publication decision with the reason.

The reason names the commit that needs a review and says how to give one.

How kix lands a reviewed head (see Design change):

  • GitHub can merge it at once: kix merges it with mergePullRequest pinned to the head.
  • The base requires the quest land check: kix queues the PR for that gate with its pin. The gate ejects a moved head.
  • Otherwise, such as while checks run: kix enables auto-merge bound to the head, entering the merge queue if the base has one, as quest merge --land does. GitHub keeps auto-merge on across a push by anyone with write access, so a writer's push after this lands with it. The maintainer accepted that gap on 2026-11-02, and GITHUB.md records it.

GitHub keeps auto-merge enabled across a writer's push, the App's included. So before any job moves or resets a branch, kix turns off auto-merge on every open PR that branch heads. That covers an iterated PR's head and a job's own branch after delivery or a restart, and stops a runner's new head from landing unreviewed. If GitHub refuses, or doesn't show whether auto-merge is on, the job pushes nothing and pauses on a decision. Branch-write tokens gain pull_requests: write for this, as GITHUB.md documents.

Decisions

Settled by the maintainer on 2026-11-01 (/quest-complete), each recommended option accepted:

  1. Which repositories count a maintainer's own ## Review record, and how kix learns it? The quest's plan says to follow a repository's policy where one names whose review counts (moq-dev/quest's CONTRIBUTING.md), and otherwise to require someone other than the author. kix cannot read prose policy. Picked A because the guide every Quest repository adopts already makes it the default; implemented in f3afefd.
    • ✅ A (recommended): count it in every enrolled repository, for write-access authors only, and never on a head the App committed. The guide every Quest repository adopts already makes this the default ("Where it says nothing, only a record on a PR whose author has write access counts"), and it matches CONTRIBUTING.md without new config. It does read the guide as each repository's policy, which is looser than the plan's "otherwise".
    • B: count it only in repositories that declare it in a machine-readable form on the default branch.
    • C: a per-project kix setting. This touches project pages, which overlap moq-dev/quest#349.
    • D: never count it, and delete the quest.
  2. The "reviewer's suggested patch applied unchanged" exemption. kix cannot verify it from GitHub's records: an applied suggestion is the applier's own commit, made by web-flow. Picked A, as GitHub's own dismissal of stale reviews behaves: such a head needs a new review, now stated in GITHUB.md.
    • ✅ A (recommended): drop this exemption for kix. GitHub's own dismissal of stale reviews behaves the same way.
    • B: plan a follow-up that matches a commit's diff against suggestion blocks in review comments.
  3. Merges that Git resolved line by line. When both sides changed the same file but Git merged it without a conflict, kix requires a new review, because it only verifies whole-file merges. Picked A: conservative until a verified line merge is worth its cost.
    • ✅ A (recommended): keep this conservative rule for now.
    • B: plan a follow-up that runs a three-way line merge in the Worker.
    • C: carry a review whenever the PR's diff against its base is unchanged. This is the rule quest/m0/helpers/review-rules.md proposes for the guide.

Also settled by the maintainer on 2026-11-01 (recorded in the PR's comments), all four review-record rules accepted:

  1. "The author's own record" means the body's latest edit is the author's. kix's App edits PR bodies when a runner delivers, so a record the App or anyone else wrote last does not count. A body never edited since opening counts as the author's.
  2. Any bot commit blocks the record. "An App-committed head" means GitHub attributes the committer to any bot account, not only this App's, since kix has no App slug configured. A record does not carry across a base merge a bot committed either.
  3. A record without a SHA doesn't count. The record must name the full 40-character SHA in the ## Review section (up to the next #/## heading).
  4. A recorded skip naming the head counts, like a review record (tested in cloudflare/test/review.test.ts).
  5. After moq-dev/quest#326, this PR uses quest:kix where the two conflicted in service.ts and GITHUB.md. Done in the merge of main.

Settled by the maintainer on 2026-11-02 (/quest-plan in the /quest-complete session), recorded in the Design change. It replaces the earlier design change that answered CodeRabbit's finding:

  1. kix landing while checks are pending. Implemented in 73213ea, which reverts e43737a's wait-then-merge path.
    • ✅ Keep auto-merge armed while checks run, pinned to the reviewed head, like quest merge --land. Accept that a later push by someone with write access lands with it. kix's own pushes still turn auto-merge off first.
    • Arm auto-merge only where a ruleset requires a review of every push; otherwise wait, then merge pinned to the head (the last round's approach).

Choices made without asking

  • Auto-merge is turned off before a job moves a branch (see Summary). It covers every open PR the branch heads, whatever their bases. A job creating its branch skips this, since no PR can come from it yet.

  • Who can approve: a person with write access, matching GitHub's required-review semantics. A drive-by approval from someone without write access does not count.

  • Where carried merges come from: the repository's default branch, not the PR's base. A contributor controls the base, so the same reasoning as the existing head-trust check applies. Merges from another base need a new review.

  • Classic branch protection: kix does not defer to it. Its review settings need the administration permission to read, so kix keeps its own check there.

  • Unreadable rulesets: a ruleset kix cannot read is one it does not defer to.

  • The check run's shape: it is named quest review, read with the existing checks: read permission, and must be written by this App's ID, completed, and successful on that SHA.

  • How a blocked merge job waits: a head that needs a review reuses the existing publication decision. The maintainer approves on GitHub and replies, then the agent delivers again. I did not add a new waiting state.

  • The fake GitHub: it gained a writers list on repositories (the seed's reviewer is one), a commit per review and per run, rulesets, REST check runs, and a general commit-to-commit compare that returns the merge base.

Limitations

  • A writer's push after auto-merge is armed lands with it, unreviewed, unless the quest land gate or an enforced ruleset refuses the moved head. The maintainer accepted this on 2026-11-02 (decision 9), matching quest merge --land.
  • Runner heads need a human approval for now. Nothing writes quest review check runs yet, and the author's record never counts on a runner's commit, so a head a runner published lands only after a person approves it on GitHub (see follow-up 1).
  • The record stays self-reported. kix cannot tell whether the review it names ran or left findings unresolved; it trusts a write-access author's word, as CONTRIBUTING.md does.
  • The approval list can be incomplete. If a PR has more than 100 writers' opinionated reviews, kix reads only the first page. This can only make the gate stricter.

Validation

  • Unit tests: cloudflare/test/review.test.ts. They cover:

    • an approval or check on the exact head;
    • a clean base merge that carries the review, and the same merge with a truncated tree, which doesn't;
    • merges that add a change, revert the base's change, or drop the reviewed change;
    • conflicts resolved by hand to either side or a third version;
    • a merge from a branch other than the default;
    • rulesets that defer, that the App can bypass, or that don't dismiss stale approvals;
    • the author's record: counted when a writer names the head in a body they last edited, and not when the author cannot write, someone else last edited the body, a bot committed the head or the merge after it, it names another commit or none, or the SHA sits outside the section.
  • Worker integration tests: cloudflare/test/publish.test.ts. They cover:

    • an unreviewed head waiting and then landing after approval;
    • no review counting from the author's own approval, a bot's approval, an AI comment from the author's account, a contributor's body-only ## Review record (no write access), another App's quest review check, or a request for changes;
    • the App's check counting on its head but not after a later runner push;
    • an approval carrying across a clean runner-published base merge, but not one that changed a file;
    • ruleset deferral;
    • a writer's own ## Review record landing at the head it names, and not at a head a runner committed;
    • landing: auto-merge bound to the head while it waits, with the hold kept while queued; the quest land gate's queue; a merge at once when nothing is left to wait for; and an UNKNOWN merge state reread once;
    • auto-merge turned off before a job pushes to an iterated PR's head, including on a second open PR from the same branch, and nothing pushed when GitHub doesn't show the auto-merge state;
    • auto-merge turned off on a job's own PR before it publishes again after delivery, and before a restart resets its branch.

    cloudflare/test/inbox.test.ts covers the inbox refusal with its reason.

  • Local runs:

    • just check passes.
    • just test passes, Worker tests included, on head 73213ea.

Suggested follow-ups

  1. Runners publish quest review check runs. After a runner publishes a head, it runs quest review outside the agent's sandbox, with the other vendor's account, on that head. It sends the result to kix, which writes a quest review check run as the App on that SHA. The agent cannot fake this, because the runner process runs the review, not the agent. This needs:
    • the App's checks: write permission, an operator step that existing installations must accept;
    • a runner protocol message;
    • a choice of which jobs run it (iterate and merge, after publishing).
  2. The inbox says up front when a review is missing. It could flag a PR whose head has no approval from its observed latestReviews, instead of explaining only after the click.

Review

  • Earlier sessions (Codex gpt-6.1-sol, medium, quest review --base origin/main): the first ended clean on head 14b236c after two fixed findings. The second, after the earlier design change, was clean on e43737a.
  • Reviewer: Codex gpt-6.1-sol, medium, quest review --new --base origin/main. This is a new session after the Design change of 2026-11-02.
  • Round 1 on head 73213ea: no findings.

Picking merge was the only gate between a runner-published head and main:
the inbox's direct merge and a merge job's landing both went ahead on the
maintainer's earlier pick, bounded only by branch rules.

Both now require a review on the exact head being landed, read from
GitHub's own records: an approval by a person with write access other
than the author, or the App's passing `quest review` check run. A review
carries across merges of the default branch that kix verifies resolved
nothing, and kix defers to rulesets that enforce current-head approval and
that the App cannot bypass. Otherwise a direct merge is refused and a
merge job pauses on a decision, each saying which commit needs a review.

The quest keeps what remains: counting a policy-named `## Review` record.

The maintainer picked counting the guide's default review record in every
enrolled repository: the PR body's `## Review` section naming the commit's
full SHA, on a PR whose author has write access and last edited the body,
never on a commit an App committed. The finished quest is removed.

Walkthrough

Merge and landing operations now require a review that counts on the exact head. The validator checks eligible approvals, a successful App quest review check, qualifying author ## Review records, ruleset enforcement, and limited review carryover across default-branch merges. Branch updates now disable auto-merge on open pull requests headed by the branch and pause publication if GitHub cannot confirm or disable it. The GitHub fixtures, tests, permissions, and documentation were updated to reflect these changes.

Merge Risk: Moderate · previous review bd29f

The new gate checks for a counting review only when a merge is requested or auto-merge is enabled. If auto-merge is enabled while checks are pending, a writer's later push can still land without a review on the new head. Close this gap, or restrict auto-merge to ruleset-bound heads, before relying on the gate.

Landing f3afefd. The maintainer's 2026-11-01 picks (1A, 2A, 3A) are recorded under ## Decisions in the body. 1A is implemented: the gate also counts a write-access author's own ## Review record naming the commit, when the author last edited the body and no App committed the head or a merge after it. 2A and 3A keep the existing behavior, now documented in cloudflare/GITHUB.md. The finished quest is deleted. The local review (Codex gpt-6-astra, deep) is clean as of round 2, just check and just test pass, and CI is green.

I've taken this PR out of the queue until moq-dev/quest#326 lands. They conflict in service.ts and GITHUB.md, where this PR should use quest:kix. After #326, main will be merged in and the PR queued again. The maintainer accepted all four review-record rules on 2026-11-01: the author's latest body edit, any bot commit blocks the record, a record without a SHA doesn't count, and a skip record naming the head counts.

Comment thread cloudflare/src/github/service.ts Resolved
GitHub keeps auto-merge enabled, and a pull request in its merge queue,
across a push by anyone with write access, and checks expectedHeadOid only
when auto-merge is enabled. So a merge job that enabled auto-merge on a
reviewed head while checks were pending would land a head a writer pushed
after it, with no review.

kix now enables auto-merge only behind the quest land gate, whose pin ejects
a moved head, or a ruleset GitHub enforces that requires a review of every
push. On any other base it merges only at once, pinned to the head; while
GitHub cannot, kix publishes the head without landing it and the job's agent
waits and delivers again. An unbound merge queue pauses on a decision.

Design change: a merge job arms a landing only where GitHub itself refuses a moved head. This answers CodeRabbit's finding.

Before, landPullRequest enabled auto-merge on any base whenever GitHub couldn't merge the reviewed head at once, for example while checks were pending. GitHub checks expectedHeadOid only when auto-merge is enabled, and keeps auto-merge on across a push by anyone with write access. GitHub's docs say it is disabled only for a push by someone without write access. A queued pull request likewise stays queued across a push (community/discussions#194832). So a writer's later head could land unreviewed.

Now:

  • GitHub can merge the head at once: kix merges it with mergePullRequest pinned to the head, as before. GitHub refuses that merge once the head moved.
  • The base requires the quest land check: unchanged. The squash auto-merge pins the head, and the gate ejects a PR whose head moved (head_moved).
  • A ruleset GitHub enforces requires a review of every push (the same rule review.ts defers to): auto-merge, entering the merge queue if there is one. A pushed head then needs a new approval before GitHub lands it.
  • Any other base: kix publishes the head without landing it and answers { head, waiting }. The runner tells the agent to wait until GitHub can merge it, such as when its checks pass, then deliver again, which merges pinned. A merge queue on such a base pauses the job on a decision, since no wait helps.

I didn't turn auto-merge off on a synchronize webhook (CodeRabbit's first option). The webhook arrives after the push, so it races GitHub merging the pushed head.

The next local review starts a new session (quest review --new).

Taking this out of the queue for a rework. On 2026-11-02 the maintainer decided that kix should go back to keeping auto-merge armed while checks run, the same as quest merge --land. A later push by someone with write access lands with it, and that's accepted. kix's own pushes still turn auto-merge off first. The wait-then-merge path and the runner's { head, waiting } result go away. cloudflare/GITHUB.md will record the accepted risk, and the CodeRabbit thread will be answered with this decision.

(Written by Claude Opus 5.5)

…nd does

Revert e43737a's wait-then-merge path: a merge job again enables auto-merge
bound to the reviewed head while its checks run, entering the merge queue when
the base has one, and merges at once, pinned to the head, when GitHub can merge
it now. The `{ head, waiting }` landing result and the runner's handling of it
go away.

The maintainer accepted on 2026-11-02 that a later push by someone with write
access lands with the armed auto-merge, matching `quest merge --land`.
GITHUB.md now states that gap. kix's own pushes still turn auto-merge off
first.

Design change: a merge job keeps auto-merge armed while checks run, as quest merge --land does. This replaces the previous design change. The maintainer decided this on 2026-11-02.

73213ea reverts e43737a's wait-then-merge path. That removes the { head, waiting } landing result, the runner's wait-then-deliver handling of it, the unbound-merge-queue decision, and their tests. landPullRequest again:

  • merges at once with mergePullRequest pinned to the reviewed head when GitHub can merge it now;
  • otherwise arms auto-merge bound to that head, entering the merge queue if the base has one;
  • or, on a base that requires the quest land check, queues the PR for that gate with its pin, unchanged.

Accepted gap: GitHub keeps auto-merge enabled across a push by anyone with write access. So a writer's push after kix arms auto-merge lands with it, unreviewed, unless the quest land gate or an enforced ruleset refuses the moved head. cloudflare/GITHUB.md records this. kix's own pushes still turn off auto-merge first, on every open PR whose head a job's branch moves or resets.

The local review started a new session (quest review --new).
